Common Signs and Symptoms of Runner’s Knee

  • Dull, achy pain in the front (anterior) part of the knee
  • Increases with repetitive movements like ascending/ descending stairs, jumping, squatting and prolonged sitting with knees flexed (commonly referred to as the Movie-Goer’s Sign)
  • Mild instability or patellar crepitus

What causes Runner’s Knee?

  • A sudden increase in activity
  • Patellar or biomechanical abnormalities such as a tight iliotibial (IT), Weakness in the Vastus Medialis Oblique (VMO) muscle, and an increased Q-angle.
  • Tightness in the quadriceps or hamstring muscles
  • Poor foot stability
  • Muscle strength imbalances in the hips, legs, and foot, that put excessive strain on the patellofemoral joint during movement

How to Prevent and Treat Runner’s Knee?
Prevent PFPS by following a proper warm-up and cool-down routine during exercises, gradually increasing activity levels, and adhering to a structured exercise program.

Treatment
After a proper assessment by a healthcare provider or physiotherapist, and identifying the internal or external causes of PFPS, a personalized treatment plan can be followed. This may include manual therapy (soft tissue techniques), modalities, activity modifications, a home exercise program, ergonomics/ postural education, taping for support, and stability. Additionally, strengthening and stretching exercises can be incorporated. Evidence-based exercises that support an effective rehabilitation plan and improve functions include quadriceps strengthening starting within the pain-free range and progressing gradually, hip muscle training (mainly lateral rotators and abductors), and proprioceptive training.
